The instinct is to blame the cartilage. The reality is that the knee is almost always the loudest joint in a quiet failure upstream.

What is happening upstream

The knee is a hinge between two systems: the hip above and the foot below. When the hip gets weak, specifically the gluteus medius that controls how the femur tracks under load, the knee absorbs the wobble. When the foot gets weak, especially the intrinsic foot muscles that should arch and stabilize, the knee absorbs the collapse.

By 35, the average person has been training the cardiovascular system in some form, but rarely the small stabilizers above and below the knee. The knee gets caught in the middle. Pain is the bill arriving for years of skipped work.

What changes it

  • Single-leg work: split squats, step-downs, lateral lunges. The knee learns to track under load.
  • Hip abduction strength: side-lying clams, banded walks. The gluteus medius gets back online.
  • Foot conditioning: barefoot calf raises, towel scrunches. The arch starts working again.
